Conditions of Use FormComplete this form prior to renting your experimental aircraft to other pilots. You must fill in all blocks. Mail the completed form to: EAA Safety Programs, PO Box 3086, Oshkosh, WI 54903-3086 Or Fax the completed for Attn: EAA Safety Programs at 920-426-6560. The completed form will be reviewed and approved by EAA. You will then be issued an EAA Authorization Letter granting permission to rent your aircraft under the terms of FAA Exemption 7162, 7162A, 7162B and 7162C. Information to operate under the FAA Exemption is found in 7162A, with the current expiration date contained in 7162C. A copy of FAA Exemption 7162A, 7162C and the EAA Authorization Letter must be carried onboard the aircraft during the periods the aircraft is being rented under the terms of this exemption.
